Community Integration Services Community Based/Facility Based

The goal of Lynndale Community Integration Services is to provide support services to adults with disabilities that enable them to learn skills required for work, community integration and independent living.

Participant asks for help in learning to:

• Take care of personal needs

• Get from place to place in the community

• Interact in social settings

• Use services that are available in the community

• Communicate with others

• Acquire new skills to become more independent and self-reliant

• Make use of ever changing technology to assist in becoming more independent and self-reliant

Community Based Services: Families of participants may request assistance when needed with community resources.

Entrance Criteria:                                                                                           

• Adults 18 years of age or older

• IQ of 69 or below

• Not a danger to yourself or others

 Exit Criteria:

• Desire to move into another service

•  Has met all outcomes

•  No longer benefits from services

•  Moved out of the service area

•  Participant  has exceeded the allowable absentee days
